A prescription for accuracy

A study of errors made by pharmacists when dispensing medicines has revealed that most of the mistakes are made when the details of the medication are typed into a computer. Researchers at Monash University, the University of Sydney and the University of Western Australia have updated the patient safety system to cut down on the number of errors.
